AGREEMENT TO PARTICIPATE IN PEER COURT
Greetings Parents and Guardians-
To allow your minor child or ward to participate in this program, please submit this online form agreement. You may be asked to sign a similar agreement later in person.
Your child has been referred to Peer Court by the SF Police Department, Juvenile Probation Departmen
t or
San Franciscoâs Peer Court program works to
help victims, offenders, families, community members and law enforcement to
come together to resolve conflicts and incidents that have happened at schools
or in the community. Peer Court is a community-based and Peer Led program
funded by the City and
Your participation is voluntary, but in order for you to participate, you must agree to the following:
1. I understand
that the program is voluntary. I can choose to participate or not to participate.
2. I agree to grant the program access to my contact information and documents pertaining directly to the incident that has been referred to Peer Court. I understand that this information is otherwise confidential, unless I grant permission to share it.
3. I agree to treat
all abide by all program ground rules, and will treat all participants with respect at all times.
Ground rules include:
a. Treat everyone with courtesy and respect
b. No fighting, yelling, name-calling or other disrespectful behaviors
c. Tell the truth.
d. Work together to reach an agreement
e. Do your best to complete your part of the agreement by the date specified
f. other ground rules as necessary or agreed upon
